I had created 5 Windows 2012 servers from a Vmware template.
And place them all to the XtremeIO datastore
2 of them wouldn't start and came up with the windows 2012 recovery mode screen.
When i place the storage on other datastores (that was not on the XtremeIO) like the local ESX disk the server started up correctly and no issues.
The solution was: adjusting the Disk.DiskMaxIOSize setting on each ESXi host from the default 32MB (32768) to 4MB (4096).
We are using ESX profile list and from the vSphere web interface we couldn't find the option and had to login into the fill vsphere client to set it.
But when you are not using profile list you could set this option under
vCenter > Host > Configuration > Advanced Settings (bottom one) > Disk > Disk.DiskMaxIOSize
